Indeed, I cannot think myself of a good third-movie title and plot.
I think he was going to bring back The Joker, but Heath's death would be screwed if he continues.
Remember: he died during post-production, TDK was practically done.

I hope he does come back. There are many other good Batman stories from the comics to use as basis for a third film, particularly of Two-Face. There's "Dark Victory" and "Faces" by Matt Wagner.
