Default Mortal Heroes and Villains

If the superhero's really DID die and did not ever get revived, who would still be alive in the Marvel U right now?

Who would be the most powerful hero/heroine and villain? What Avengers would still be alive?

Nightmare killed everyone on earth before, so no one on earth would still be around. So probably gladiator or hyperion for the MU. For DC it would probably be one of the lanterns I would guess. Most everyone's died at one time or another. Assuming we aren't counting cosmic beings of course. For villians we'd probably be looking at Kang for the MU and Superman Prime I would guess.

Let's forget about the outrageous mass murderers who have the ability to change the universe and let's also disregard the ultimates universe. I'm talking about Earth-bound heroes, villains, and mutants.

What would the Marvel universe look like if the major Marvel deaths remained permanent and the characters were not revived? Who would still be standing, who would be the biggest villains? What would change?

Beta Ray Bill died at the end of Stormbreaker, but was saved by some mysterious force and bonded to Simon Walters.

Based on that rule the most powerful earth bound hero would far and away be Doctor Strange easily.

Doctor Strange must've gotten killed off at some point. I would be utterly shocked to find that any of Marvel's first few tiers of characters, from the big guns like Cap and Spider-Man to the moderately recognizable guys like Hawkeye and Strange, have not been killed off at some point in the decades they've been around.

If we're only talking earth type threats then no he hasn't. Any death by him has come at the hands of cosmic threats.
