Default Comic Artists Commission Fees?

I was curious about various comic artists commission fees. Some do it through the mail, or online,
I found this website that listed Mike Allreds:

Alex Ross?
Ryan Ottley?
Jim Lee?
Alan Davis?

If you know the commission prices for any artists please post them here, and possibly where to contact them for commission.

At the Comic Con I found out,
Erik Larsen draws a 17x11 full figure and inked for $200
Francis Manapul draws and paints a 17x11 (I think) cover image for $500
Mike McKone will draw and ink a head shot for $70 or a full body for $200
Ed McGuinness will draw a full body character for $500

It will always vary according to artist, artwork size and finishing method (pencils, ink, paint,etc.)

What I wonder is how much their commission is for actual comics.

There's no way they're making $1,000 dollars a page doing monthlies.
