World Captain America vs ...?...

please help me, i need some info about a story that's probably the first captain america story i read (ages ago) ..i remember only that he was fighting a ..deformed man. this guy was hunchbacked just like the hunch back of notre dame and wore only hulk-like trousers, the skin was red like burnt but the most noticeable detail was that he had this strange long mouth dangling on a side... that's all i remember. i'd like to know who was the guy, the title of the story, maybe some pictures.. thank you!
