I still think it's Sara. Oliver "choosing" Sara over Shado is what started all of this.

Sure, Slade had the chance to snap her neck at Team Arrow's lair, but I think he didn't do it then because the timing wasn't right. Her death would've been pretty sudden and that's not what Slade wants. He wants Oliver to know he's going to kill her and that there's nothing Oliver can do to stop him.
