Ah, I figured. Well, I just spoke to someone on Reddit who said he had fangs, but not as rat-like, and some of his teeth have fallen out.

As far as what they were responding to in the second photo:

Apparently Orlok seems to have some form of the fangs (chipped/jagged teeth??????), but he's missing a lot of his teeth, which would explain why everyone seems to be seeing something different as far as the fangs go, i.e. fangs vs. being toothless.
