IMO, it depends how he pulled it out. If he wiggled it out, the yeah, it would cause damage to his heart. If he pulled it out smoothly, then I think he would be fine?
The most critical error occured when Irwin yanked the barb out of his own chest after being struck, leaving a palm-size hole. Like a double-bladed dagger with tiny fishhook teeth, the barb caused much more structural damage to his heart when pulled out than when it went in.
