I would remove the arm that's lower down and further to the front. The other looks like it's positioned more normally.

I wouldn't remove it if other kids had a 3rd arm. However, since he is "one of a kind" it's better to make him as normal as possible. Otherwise he may be ostracised by his peers.

If they do amputate one of the arms, they should put it in a jar of formaldehyde for him as a keepsake for when he gets older. He can pick up chicks when he gets into high shool by saying "Wanna come over later and see my third arm?"
