Aside from that, Simone isn't beside Isaac.

And if this is an early promo shot, we won't have known about DL, thus it would be better not to give the family relationship away via positioning.

You can tell from their outfits and such that it was taken when they filmed one of the versions of Peter's vision of him blowing up.

Dr. Suresh was leading in the immunity challenge when a fatal miscalculation took protection away from him. Fellow team mate Peter tried to salvage the challenge only to get his emo haircut removed. Earning both he and Suresh an emergency chopper ride off the Island.
