Considering it was the wife that drove the car into the river, I'd save the child.

As I was getting out of the car I'd probably have to turn to my wife one last time and say "Good one."

Although, i would leave the door open so the wife would have a common sense to unbuckle herself and swim out also.
If she's alive, yay.
If she died in it, well ...
