Call your county court house first and they will tell you what to do. You will have to show proof so a birth certificate and your license and ss card will probably work if you have to go in to pick it up. You could also call the SSA like someone mentioned and tell them to mail you another one but I doubt they would give it over the phone.

I lost mine years and years ago, I told them, and got it 2 weeks later in the mail.

The above website is what you can use to find out what all you need to do. It looks like you'll have to reapply if the SSA office shows no record of the original application. You can get a Form SS-5 at the above website. If you don't have an SSN for the kid by April 15, you can file a Form 4868 (which is an extension request form) to extend the deadline of your return. You can find an online extension form here:

You need to contact the Social Security office for your state....you have to provide them with the birth certificate for them to process it.
