Per Morg's suggestion I did PM each of the Moderators/Admin to request their own personal procedures for Probation/Warning with the advent of profile and thread probation.
I'll post them here as the Mod/Admin gives me permission to.
Thank you to the Moderators and Administrators who answered and are allowing the information to be posted.
Each Mod/Admin is able to interprete their own procedures as they wish, this is just for the general knowledge and information of the forum

I will be using thread probations for people who post spam threads and/or repeat threads after being warned not to. Profile probation will be given to those with offensive sigs or avatars, or those who continually break the 5-6 line sig rule, or use it to insult and harass other posters. I reserve the right to use these methods to punish persistant problem makers who disobey other rules than this if I think it will get their attention to the fact that they misbehave.

Was there a change in the way warnings/probations were/are done?
I didn't see a change in the FAQ's about it, nor a global sticky.
I've seen the 'thread probations' before, like once, but it was never added to the FAQ's and the Avatar thing is new and I haven't seen that announced.
What's the new guidelines for getting thread/avatar regulations as a type of 'probation'?
What are the infractions which would cause these types of probations?

Warnings
Administrators and moderators use warnings to help troubled users. Warnings give users a chance to modify their posting habits before probation or banning. Warnings are not required to be issued before probation or banning occurs. This is at the discretion of the moderator or administrator taking action at the time.
Probation Duration
The length of probation is up to the admin/mod taking action at the time. Typically seven (7) to ten (10) days for initial infractions. For major infractions, the account may be under probation for longer periods, or banned immediately. For returning to the boards after being banned and continuing to break the rules, you may either be banned again, or have your IP banned and/or your IP provider may be contacted.
Violations of Probations and Banning
If you are on probation, you may not post under any account for the amount of days for which you are on probation. Violation of this probation will result in the permanent banning of your ORIGINAL account (permanent loss of entire post count, avatars, etc.,) and an extension of your probation to your NEW name. Continued violation of this rule may result in IP banning. If you are permanent banned, you may not return to Hype. Note: the Administrators and Moderators can tell if you hold more than one account.
