I remember reading it somewhere but I don't know where.

In "Going Sane" he assumed that name but it was never his real name.

Never. The name was itself merely a joke.

It's a good one though. I like the idea that his name has always been Joker, one way or another.

End of BB when Gordon hands him the card, the evidence bag says "J. Kerr"

That was supposed to be the name of the investigating officer that found the card. It was indeed a nod to "Going Sane".

It was also used in Knightfall.

"Who's there?"
"Joe"
"Joe-who?"
*smashesdoor*
"Jo-Ker"

Actually, I don't know if this is the same instance, but I do know that in DC's comic-book adaptation of the '89 movie, the signature "Joseph Kerr" is inscribed in place of the Secretary of the Treasury's name on the phony money he dumps on the crowd at the parade (his portrait's on each bill, too).

His name is often cited as Joseph Kerr, but his actual name (along with many details of his earlier life) are unknown thus far.

Then where did the Jack Napier name come about?

Jack Napier was solely Tim Burton's invention.
