Help w/ Book/Game That Needs Identification

For the past ten years, I have been trying to identify and purchase another copy of a game that my mother purchased for me when I was young. It was an activity box of sorts and the only lead I have is one of the cards, which I have linked below. Somehow during our move, it was misplaced and I have been wanting to solve this problem for two years.
