I feel like he didn't have a complete arc in season 3, and that the writers forgot about him. I hope to see him return an fight with Team Green Arrow but in full costume this time. The bank robber mask doesn't do it.

The actor became the lead on another show, as I recall. It is a shame that his arc was truncated. I think it'd be awesome if he appeared in the future and helped out Team Arrow. With everything going on in Star City now, it'd be great to see Ted going up against some Ghosts (and commenting on how much Laurel's fighting skills have improved would be most welcome, too).

It's a loose thread, but honestly, I didn't care about him when he was on the show, and I care even less about him now that he's gone. It doesn't matter to me if we never see him again.

I thought it was pretty clear based on the dialogue that he was dying. Laurel said something like "you're going to be OK" and he responded with "at least you're a better fighter than a liar."

I thought it was referenced that he survived, though. And, I mean, Brick beat him pretty soundly, but it didn't look like anything fatal to me.
