It's definitely special effects, ala Benjamin Button. I think asking Evans to slim down considerably in such a short time and for such a short part of the movie would be more than excessive.t:

That is an interesting take. There were rumors that Nicholas Cage was on set during the filming of the transformation scene, giving Evans tips on portraying a character going through immense amounts of pain...I'd like to see the scene play out like this..
He's in the tube and the power turns on.
Steve - "ZOMG! It burns! It burns!! Make it stop!! i'M DYING!!!! I can feel my skin melting off my body!! WHYY!! WHYYYY!!!!!!!!!!!!!!!!!! Hey...this feels kind of....AHHHH!!! NO! IT'S KILLING ME!"
And then the door opens.

I'm just hoping that the presentational aspect of the transformation isn't skipped over. It's a dramatic moment, I'd like to see it built up a bit. A bit of theatrics if you will, especially up until the moment the machine is turned on.

Something just dawned on me...and everyone else probably already figured it out. :P So, the maker the SSS is killed. Which means the only way to make a STABLE SSS is gone with him. That doesn't keep the government from keeping unstable copies of it in cold storage. In The Incredible Hulk we see the General give Blonsky the SSS.
Obviously, the General must have known how unstable the SSS was and how it would likely effect him in a very bad way, but out of desperation for wanting The Hulk, he did it anyway. Proving just how obsessed he was with capturing The Hulk.
I'm slow...I just now realized his desperation after reading about Cap.
