Grillz were a big thing in the South but managed to stay there. It wasn't until rappers started wearing them that it started to be seen in other regions. The Nelly song "Grillz" also was a huge contributor to the grillz fad

It's just another way to wear valuables, and appear more wealthy. I don't think it's supposed to necessarily be attractive (I agree with you though).
