I think Alexandre Aja definately has the market cornered with not only gory, but realistically gory deaths. The decapitation of the father in High Tension by the use of a rolling piano after he's had his head brutally wedged through some stairwell banisters and the extended scene of the father roasting alive in The Hills Have Eyes both make me wince a tad. He may also have a daddy complex or something too...
