Didn't there was more than one issue where Doc Ock knew Spidey's identity?
The one I'm talking about is when he saved Spidey from some kind of poison, and I think he was with Titania or some other chick.
 
Web of Death. Ock was killed at the end of that story. He was resurrected by the Hand usin' his engrams planted on a chip in his bid to become a virtual reality villain known as the Master Programmer. Of course, that data hadn't the time to be updated before his death so he never knew Spidey's ID.
 
Web of Death arc in the Clone Saga. Good story with a sucky ending.

Ock saved Spider-Man from a lethal virus he was dying of, because he wanted the pleasure of killing Spider-Man himself in his prime. Basically he felt cheated out of not being the one to kill Spider-Man.

In the process Ock unmasked Spider-Man and learned his identity. However he was killed shortly after by Kaine. Ock was then ressurrected a short time later by a mystic cult known as The Hand. Ock's memories were restored into his mind. But it was only his memories up until before the Web of Death business.

Hence why he did not know Spidey's identity. Btw, his girlfriend was called Stunner. Hope that helps :)
 
That explains a lot, but how exactly did he came back to life?
 
They used a mystical ritual transferring Stunner's life force to Doc Ock's body. Or something like that.
