Well i only say that b/c a scar like that would probably not look so blended in with the skin, and also i just wouldn't see nolan showing joker int he movie with NO makeup on at all. I know he is making him a guy who paints his face but i think he also understands the mystery of it by keeping it on even in different layers
it wouldn't make sense to put flesh makeup over white makeup which is over his real skin, it's redundant imo
even if you examine his skin in the picture, it's very sallow and pale, showing how his sin has reacted to being covered by makeup for such an extended period of time.
And i believe nolan would show the joker with no makeup, because
1) it's a homage to his first appearance in the comics when he dressed as a police officer (it was not stated at the time that his skin was permawhite)
2) Nolan seems to be focusing more on the joker's internal workings than his external appearance.
