I got warned by Morg recently for having eight 1-font lines in my signature, as opposed to six. Never got a verbal or anything, just the warning. But I think I'll avoid my choice words on that.

Closest I've gotten to being banned was at MFG Forums. The Staff was talking to the Board Founders, and during the convo one of the Founders started pulling rank on me to get me to shut up. I told him in a less-than-friendly way that he could de-mod me at any time and stick the ranking business where the sun doesn't shine.

Needless to say, he didn't take it kindly. But no, I haven't been banned...yet.

I don't think any of us of stable mind ever intend to offend anyone else. But that's the Achilles heel to moderation- every moderator has a different stance on what does and doesn't offend them just like every user can say the same. Thus, you need to guage two different reactions to the same material- a precarious line to tread, to be sure.

I got banned for about a weekend for posting a picture that was obscene, really obscene. It's an interesting story about linking on the internet. It was a huge misunderstanding and I really felt bad that I might look like a huge jerk, when it was really just that I'm not very smart.
