It never explained it. The good doctor did perform surgery on Ponda Baba, however he botched the operation which incurred the wrath of the Aqualish upon him.

Then the "... apology accepted.." guy who crumbles to his knees, don't know if that was Piet?.

By the way, there are plenty of mistakes in the trilogy. For example when Luke & his Uncle Owen purchase the droids from the Jawas, Owen initially purchases a R5 astromech droid (R5D4), to which Luke calls an "R2 unit".
