Regarding Lauren: Shelby has a deep-seated hatred of fathers, who are all men. He felt abandoned by his mother, but he didn't hate her the way he hated his father. I think they implied he even visited her in the hospital. Shelby also doesn't try to kill Madison until she stumbles literally right into his secret and knows for a fact that he's the Origami Killer.
So the fact that Lauren is a woman probably explains why he didn't instantly kill her when she started nosing around. After that, I think their relationship was quite genuine, at least on some level. Shelby seemed to like her and they grew close, so he was even more reluctant to kill her.
And, of course, when Kramer tries to kill both of them, you do have the option of leaving her to die there, if you want to play Shelby as an amoral killer.

As for sequels, I think QD should make a new game with a new plot and new characters, but leave it in the HR universe. Maybe have some nice easter egg references to Jayden's latest case in the papers or something, but I want to see what else they can come up with rather than milking HR for more.
