Most unsettling moment in a game

One point I dread in any game is in MGS3. Where you're walking down the river and the Sorrow tries to blast you, and you get those unsettling images of him and that horrible scream!

I still own the Mega Drive (Genesis) and almost every part of Echo The Dolphin scares me. But mainly after you jump out the Ocean and everything in it gets sucked out and you're left alone with this creepy music.

Giygas from Earthbound. It's bad enough, but when he starts speaking to you and the images of a fetus start emerging from him!

SH4. Where you wake up in the Hospital World, and Walter is performing a hysterectomy on a monster/woman thing. Then he starts slowly walking towards you with his hands covered in blood!

Arkham Asylum. The part in the morgue is just creepy as hell. Thomas and Martha all zombified, and then when you open the last bag, out pops Scarecrow, followed by his spooky world and a giant version of him!!!

Arkham City. The Mad Hatter's tea party!

There are more, but those are the ones that made an impact on me.

Thats a great one. What a weird level that was. Also those audio recordings of the mad hatter were just creepy.

Super mario 64 creeped me out at times too. Like the giant eels. And just the general lonely feeling.
 
In arkham asylum, for some reason, the level when you're in the catacombs looking for these spores, and you have Killer Croc hunting you, and it's completely silents, just the sound of the water swaying around. gave me the heebedy jeebedies.
 
The Sims- The very first time I trapped a Sim in a room to watch him slowly die.

The whole broodmother thing. What Branka did... seriously, seriously f**ked up. Haunting stuff.

Hahah yeah, I think that's the main reason why I'm so freaked out by open water. In games and real life.

It was creepy. The way you see him and his goons at the end of the table. Then Batman screaming with that rabbit mask on!

I still get creeped out by the the Big Boo's haunted mansion, and that maze like area underground lol

I don't often play horror type games and when I do (Deadspace, Alan Wake) I never take them seriously, so I've probably missed out on a lot. Only one moment really stands out in my gaming experience; the first time I ever saw a Centaur in Fallout 3. Turning to see one less than ten feet away as it made that weird noise, wandering toward the player slowly -it definitely gave me a moment's pause.

I got used to them soon enough, but prior to my first encounter I was completely unaware of them and the first sighting -especially with it being so unnervingly close- was very much unsettling.
 
In light of the re-make (which is getting great feedback with critics half way through the game so far) X-COM UFO Defense is still one of the scariest games out there. There is nothing more unsettling then the ambient music (download it it will still give you the creeps) and letting your squad go deeper into the darkness with aliens jumping out at you. The tension of that game still exists today. Regardless of the graphics it still makes you bite on your nails and yelp out loud.

I can't wait for Enemy Unknown in a few weeks. XCOM games are great if you want to be scared half to death with your jaw perpetually clenched.
