I'm guessing that Maya Hanson and Ellen Brandt may have survived due to the Extremis healing factor. Their roles in Iron Man 3 were cut substantially due to Perlmutter's interference and insistence that, "women don't sell action figures." He no longer has a say with Marvel Studios and A. I half-expect that Maya used Extremis on herself and B. Ellen is needed back if Marvel ever adapt Man-Thing.
Ronan the Accuser is likely to return as a clone since the Kree are all about genetic engineering.
Ultron is almost certain to have a backup somewhere. He'll be back.
