Yes. It happened with the Buffalo Sabers a little while back if I am not mistaken. That and the Montreal Expos, MLB had to come in and run the team.

Good question, though that isn't the worst placement of a team in a division/conference ever. That belongs to the Miami Heat, who were in the Western Conference for their first season in the NBA.
