Alright, so my natural curiosity and love for all that is twisted and wrong has led me to look up a bunch of medical factors that would make up "The Joker".
I figure If there was a person out there with a sick enough sence of humor and distruction that has "Labile affect" and took a **** load of "Haldol" we could wind up with someone just like the Joker,...so he's posable.
Labile affect- or pseudobulbar affect refers to the pathological expression of laughter, crying, or smiling. It is also known as emotional lability, pathological laughter and crying, emotional incontinence, or, more recently, involuntary emotional expression disorder (IEED).[1] Patients may find themselves laughing uncontrollably at something that is only moderately funny, being unable to stop themselves for several minutes. Episodes may also be mood-incongruent: a patient might laugh uncontrollably when angry or frustrated, for example.
Labile affect is most commonly observed after brain injury or degeneration in amyotrophic lateral sclerosis (also known as Lou Gehrig disease), a form of motor neuron disease.

and in anoth note, Harley Quinn.
...check this out,...if anyone ever wanted to have a twisted/realistic version of Harley, you might be thinking of this disorder.
WARNING: not for the faint of heart
