One of the worst I remember was The Matrix Revolutions:

I mean, Jesus, couldn't the guy who designed this taken some time to at least mix all those images together?

But that Dead Silence one tops it--it does look like they just slapped some word art on an image from the movie and called it a day.

I made a great cover for the Soundtrack for my Graphics Arts class when that film came out.
It's a pic of Neo standing, looking out at the machine conquered Earth. Infront of Neo there's lightning striking in the distance. In the lightning, I put the title in Matrix letters.
I have to go dig it out so I can show you guys.

Real Genius is a classic by all means! One of Kilmer's best.

Real Genius is awesome, but every time I see that cover I can't fathom how anyone approved it for the DVD release. Good grief.

I have replaced my Jurassic Park cover with one that was all black with the logo on the front and IMO it looks a lot better.

To be honest I don't think the Dead Silence cover is horrible at all, just simply uninspired. That Children of Men cover, on the other hand, is trash. Seriously just god awful, no getting around that.
