It would be cool if he were tortured by someone like Baron Strucker when he was younger. Maybe he was interrogated and refused to give in and then had his eye gouged out as punishment. This could set up an enemity between them, perhaps for a future Cap or SHIELD movie.

Anyway, I think it would have been nice to see the eyepatch explained in CA, otherwise I don't really think it's necessary to have it explained.
