No, she didn't. She snapped her neck but kept her alive, then left her hanging, alive, over a Lazarus Pit. If I wanted to kill someone, I wouldn't just cripple them and leave them hanging over a pool that's specifically designed to heal all wounds. Shiva never died by Batgirl's hand. Batgirl just showed her very graphically that she could kill her, thus fulfilling Shiva's death wish.
