The Ragged Man, Book Four of the Twilight Reign by Tom Lloyd.
Currently the series' protagonist-Isak-is a hollow shell of a man, due to the torments he suffered in the pits of Ghenna (Hell). His bondsman, a harlequin, is trying to cope with the fact that he had to pledge his soul to the demon tormenting Isak in order to find a way to escape Ghenna. Soon Isak will discover that his wife sacrificed herself in Ghenna so that he and the Harlequin could escape.
Also, the antagonist is expanding his empire and amassing power enough to slaughter the Gods. A shadow reborn as a child is sending out harlequins as prophets to turn the people's worship toward Itself, away from the Gods. The Gods are pissed off, causing their closest follows to become consumed by a holy rage.
Very good if you want something epic and world-shattering.
