Achievements that just don't appear

I know this isn't just a problem with me, but just off the top of my head, while playing Fracture, Gears 1 and now Halo 3, I've got on to the next level, without getting the achievement for the previous level.

Is it a problem with the game? Am I doing something wrong?

If you're playing Halo 3 on Easy, you won't unlock achievements. I think the same applies for ODST.

Gears & Fracture could be glitched for you.

Yeah, I'm playing it on Normal, specifically for achievements. Probably glitched too.

I've found that if I use my profile on another persons xbox then go back on mine without reloading my profile that it won't give me achievements.

Rainbow Six Vegas 2 still owes me at least 85 gamer points that I earned and never delivered. I'm still waiting for that "patch" they said they were working on

Yeah, RSV2 owes me the achievement for shooting people while on the rope.

I've heard people have not been getting the Realistic achivement for Splinter Cell Conviction...but I think it's been fixed.

I'm happy to say I haven't had this problem before.
