Digital stunt-doubles are always useful, and I've no doubt that may come into play for Joker as it did for Batman in BB; but I also think they can't do that until they know what the Joker's going to look like, and it's a foregone conclusion that one reason for scanning Ledger would be to work on makeup concepts.
As others have I said I doubt it'll be heavy prosthetics but i wouldn't be surprised if they modified his nose and chin to elongate his features some (without going apenuts, of course).
It's possible that some CGI would be used to augment his features in some instances but since Nolan doesn't like to use a lot of CGI I would doubt that they would use it much for Joker's appearance. However it would be interesting, when he smiles (which would not be all the time) if the smile was subtly CG-enhanced to make it larger...
