I noticed in that video the girl Joker steals the drink from is seriously freaked out. That's not acting. That was the extra's natural reaction to Heath in full Joker mode. She's honestly terrified.

This is exactly what I thought happened too. If you look closely Heath has a little smirk/smile just after he does it. t:

My most favorite line is when he tells Batman "You complete me". That is classic !I laughed out loud at that .
Heath Ledger delivery of a lot of the lines made them great lines . He didn't really say anything extraordinary it is the way the tone of his voice would change .Or the way his eyes would shift . Pause between words . I can't say enough how great he played the Joker .

There are many great quotes from Joker, but I think it's not just the words he said, but how he delivered them that made them memorable. Like when the mob boss asked him "Do you think you can just steal from us?", the Joker paused for a second, his body language was one of matter-of-fact, and he replied "Yes". That was a perfectly delivered line.

I have the book, and it doesn't say he mouthed anything which means that Heath added that himself. In fact after reading the script, I think most of the stuff he says was improvised the whole "I'm not. No...I'm not!" and "Ta Dah!" aren't in the script.
