Before I state what I'd like to see in the third film. Here's a rough treatment of how I'd like to see in the opening five to 10 minutes :-
The opening shot of the bat logo is important I feel. The one in the first film related to Bruce Wayne becoming the Batman (surrounded by bats, overcoming his fear) hence the swarm of bats. The second was clever in that the one of the Joker's tenets was "everything burns" hence the Bat symbol emerging from a fireball. They're all symbolic of course.
I think the third film should start with a black screen (with those musical drum beats of course), each beats coincides with a 'sonar ripple'. In each ripple a symbol emerges more clearly, it is the bat symbol. The brightness of each sonar intensifies, on the final one it turns into a bright light. The camera then zooms out of this bright light, revealing a bat superimposed on the moon..

It is momentarily suspended in front of the moon. Then this bat swoops down into the well that leads to the batcave. The bat continues to fly through the batcave, weaving through the natural stone pillars (reminding us of his origin). Other bats join it, then they suddenly shoot out through the waterfall. We then focus back on the same bat, as it flies towards the centre of Gotham. The bat swoops down towards Gotham, it and some of the others perch themselves underneath a motorway bypass. We see the original bat looking down the four-lane motorway. It seems startled and flies off. As it stretches its wings, we alternate with that shot and one of Batman's cape billowing in the wind. The camera then pans along the Batpod, we sees it wheels turning, Batman's cape billowing, the background is out of focus...(as we see this, we only hear the Batpod passing traffic, nothing else)

We then hear the words of Commissioner Gordon: "Because he's the hero Gotham deserves..so we'll hunt him because he can take it...he's a silent guardian, a watchful protector. A dark knight.". (I purposely truncated his speech)
Just as he ends with "a dark knight"...

We get the 'lights on' sequence with the Batpod and the sounds of the world around it come flooding in. Sirens wail, the roar of the turbo engine from the Batpod charges up. We then see several police vehicles giving chase. Their CB radios abuzz with activity, voices frantically exchanging words. "We're in sight of the vigilante Batman.." "proceed with caution, he is armed and highly dangerous", "he's on a vehicle", "what?!" "I said he's on a vehicle" "make and colour", "I dunno....it's a fast..black..bike!" *loud turbo roar* as the Batpod whizzes past. We see a HUD (heads-up display) on the panel below him. It pulses (like a sonar) showing a direct route to the batcave. "Force him down Willard Avenue...we have a surprise for him there". Commissioner Gordon: "do not shoot the Batman! I want him in a holding cell, not a morgue!"
I'm no scriptwriter but that's the essence of what I had in mind.
I'd like to see a more bulkier Christian Bale. I think he had the right muscle mass for the first film but seemed too slight in the second film.
I'd like to see a batsuit that is not so small. Whilst the first suit wasn't perfect I liked how it gave Christian Bale a more imposing presence. It was described as giving him a more 'animalistic' quality. I like the logic of why he had a new suit but the suit itself didn't work for me. The challenging aspect of designing a new suit is appeasing both the need to be armoured against knives and bullets and the need to be fast and agile without encumbrance. Lindy Hemming may have her work cut out!
I would like the film not to stray from Gotham city. The Bhutan scenes in the first film made sense as Bruce Wayne had to undergo that journey across the world as it were. However, in the second film, taking the action to Hong Kong was not necessary. Perhaps, Christopher Nolan can explore more of Gotham's underbelly. Revisit the Narrows, witness the further decay of Arkham Asylum and see more of Gotham at night.
I think the Black Mask/Roman Sionis would be ideal (from my point of view he is a must as I do not see another suitable villain). I feel he would tie-in well with the theme of identity. Which I see as a possible main theme. I see the Black Mask/Roman Sionis as a perfect mirror to Batman/Bruce Wayne. To me, the Joker was only an opposite to Batman. Ra's al Ghul was more of an extreme version of Batman. In some ways the same as Two-Face.
I'd like to see a new design for the batmobile. I love the Tumbler, I think it was a stroke of genius by both Christopher Nolan (he made a basic clay mould - which was inspired by the cars in Blade Runner; as was the look of Gotham) and Nathan Crowley (production designer). The latter was the one who physically had to design then create the Tumbler. Kudos to him.
I would very much like to see the batboat. Now it would be tempting to make the new batmobile a hybrid vehicle. Although, we're already had the Batpod emerging from the Tumbler. So any thing remotely like that would simply be a repeat.
The score will be important too and I loved the score for both films. The score for The Dark Knight was clever in that it expanded on themes that were very briefly touched upon in the first film. For example, the My Otis track from Batman Begins, timing 3:40-52 (Hong Kong window break-in) and timing 4:04-18 (Harvey Dent theme). Whilst the score is phenomenal, it was nearly absent from the second film and certainly didn't mesh well with the action on screen. The only times I remember the music making an effect was at the beginning with car park scene, the Hong Kong break-in and when Batman jumps off the roof to the Joker's building.
I would like to see Bruce Wayne and Batman for that matter back at Wayne Manor. I would like to see modifications to the "south-east corner".
I'd certainly like to see the batcave in use once more. Which was sorely missing from the second film. Things like the Tumbler and the batcave are extensions of Batman and help form his character. "Mind your surroundings."
I would like to see far more emotional engagement in the third film. Some thing which was at the core of the first film (Bruce Wayne's slain parents and him coping with loss as a boy). The problem with the second film it was overladen with ideas (some clever ones) that got in the way of conveying any emotion. I didn't feel any thing when Bruce Wayne sat there crying about Rachel Dawes and his failure to save Harvey Dent from disfigurement. I didn't either see much of a chemistry between Bruce Wayne and Rachel Dawes.
I would like to see a change in the fighting choreography. I felt it was too slow and seemed staged. At times there was no fluency in the moves. I'd like to see a greater sense of urgency and perhaps more brutal hits. I understand why the fighting was slower as some complained about how the fighting was too quick in the first film (a fair observation but not one with which I agree). Perhaps, an ammalgamation of the two would be ideal? I believe it was the Keysi fighting method.
Finally, I'd like the third film to focus on Bruce Wayne/Batman. His inner turmoil, angst, depression and perhaps nearing breaking point? I thought the second film lost focus on Bruce Wayne/Batman and so made him a less interesting character. I would also like to see how Commissioner Gordon finds leading a hunt for Batman whilst knowing he is innocent, this will surely press on his conscience.
