Hitting MJ was completely accidental, and probably could have still happened with or without the symbiote.
 
To each his own, I guess. Since MJ was the closest to him, I can understand why it would be hitting her that would be his wake up call.

As for killng, maybe it just didn't set in right away? I mean, he was caught up in the moment when he attempted to kill both Sandman and Harry.

I can understand him being caught up in the moment, but he should have realized it after the fact. Hell, he never seems too broken up about it even after he gets rid of the symbiote.

Like I said, him hitting Mary Jane is completely accidental, not something the symbiote drove him to do, like the (attempted) murders were. The situation would play out the same way even without the symbiote's influence.

Well, remember, Peter's rge was fueled by the Symbiote. At this point, it pretty much had completely taken him over. Peter was much less responsible with the Symbiote as well.

But he was reacting instinctively to an alert from his Spider-Sense. The symbiote had nothing to do with that.
 
But I think attacking his friend with intent to kill should have been enough for him to wake up. I mean, by the point he accidentally hits MJ, he's already murdered two people.

That's actually what I've felt too. Peter just got lucky that the 2 people he "murdered" didn't actually die. But that should've been enough to make him realize how dangerous the symbiote is. Btw,I'm not sure he actually hits MJ. It looks like he just violently shoved here out of the way(without even knowing it was her).
