Well, if there's one thing that I would tolerate the Joker doing to himself, it'd be the red lips. In The Dark Knight Returns, for example, he didn't have red lips until he put on lipstick (not quite canon, but at least there's precedent).
And it'd be more realistic too. If you accept that a guy's hair and skin would change color due to chemicals (not unreasonable), it would seem that all his skin, including that on his lips, would be turned white. So, it seems to me that the hair and skin were the original deformations, prompting him to notice "hey, I kinda look like a clown now. Let's add the finishing touch".
Anyway, about the pic. What I'm noticing is not the "cut smile" that people seem to be calling it, but rather a scar that happens to be next to his mouth. So what I anticipate is that when he actually smiles that Ledgeriffic smile of his, the scar will highlight the stretching and creasing of his face in such a way that the smile will look extra freaky and evil, without actually deforming the mouth itself.
I'm looking forward to it.
