He wasnt grinning, it took me a minute to figure it out too, if you really look at it you can see were light/texture differs slightly enough

Yeah the smile was a close up on the face. the body was just as Cap says "Hulk" and he turns to Cap, and then Cap says "smash" and Hulk THEN smiles but it's a close-up.
There never was a shot like that futher out.
