What are we thinking this "jon doe" did to get locked in Arhkam ?
Armed robbery, double homicide...got a taste for the theatrical. Leaves a calling card.
You might want to look up photos of what a congenial disease can do to a person. Or not, because admittedly it is some very intense stuff. This is all very much in line with that.
Plus Matt Reeves is generally pretty transparent and straightforward with fans when it comes to this sort of thing. If he's highlighting that this is the result of a congenial disease, then I believe it.
Uhm...
Yeah, but what about green hair?
I haven't obsession for explainations and ultra-realism, but I think Reeves is going to an almost realistic way to characters even if horrorific.
So, okay skin messed up and stuff, but... Green hair?
The green hair is probably dyed. Especially since it's only tinges on the frontmost hair on his head, as it seems like John Doe's natural hair color is some shade of dirty blonde, judging by his eyebrows and what hair we can see on the backside of his head.

I just don't see how this disease has a very pleasant personality.

You're probably right.
But if that's so, why did he dyed is hair green if he hasn't created yet the Joker persona?
I guess we'll know it in Arkham series.
Anyway, now it's clear that Joker is heavily inspired to Elephant man, and considering the scene screams Silence of the lambs all over, I definitely think we're gonna see this dynamic:

With Joker and a doctor... a strange doctor...
I can't say that I'm a fan of the this design for the Joker. Despite where the influences might come from, it's too much. He looks more like a zombie. I feel like it should be dialed back a bit.
With Joker and a doctor... a strange doctor...
Isn't it more likely they'll use Harleen Quinzel as his therapist? You're probably right.

I would totally be on board for this. Plus, I think a fedora would fit this Joker really well honestly. What are folk's thoughts on his Joker getup once out of Arkham?
With Durran not being a huge fan of more anachronistic costuming, I love an adaptation of the pared down, more modern purple and green suit, Neal Adams gave Joker after Denny O'Neil rebooted him back into a maniacal killer in the 70s:
