There are some really good ideas here especially concerning Black Mask. For me, given the events of TDK, its not the direction that I want the next movie to go in.
To me the 3rd movie should fulfill a prophesy from BB when Ras said Bruce would become a legend. So the 3rd movie definitely needs to rap up Batman status as a fugitive and Bruce coming to grips that he is more Batman than Bruce Wayne and that his mission will be neverending one.
I would like to see a more intimate movie that is more of a pyschological thiller than action/adventure. I don't want Nolan to try to outdo TDK because I don't think he can. So I would take this approach. That is why I would rather see the Riddler and Scarecrow as the villians. The Riddler I see as more of a cross between the Zodiac killer and John Doe from Se7en. Scarecrow should come back with a new toxin that along with the actions of the Riddler takes Batman (and the audience) into a literal pyschological trip into his mind.
I would love to see a surreal sequence involving the fear toxin that is more Watchmen or Sin City in appearance that plays out like Batman from the DC Universe.
Of course I would love to see a Selina Kyle but then the dilmema is how do you balance 3 villians without taking away from Bruce/Batman because this should definitely be his story and no as much of an ensemble movie like TDK.
