Yes that first Arkham book really would be R rated, but MOST of the Batman universe is PG-13.

It really isn't anything to do with scare value its more like, hmm disturbing for some. I mean how often do you have a mother eatting beatles and chances are if that were translated onto the screen it would def be enhanced for todays modern audiences.
Hell, decapitated heads don't even warrant R-ratings anymore! In LOTR, Fellowship of the Ring, one of the Orcs gets it's head chopped off and it's headless body stands for a few seconds before falling to the ground. And if that isn't enough, in Return of the King, there's a scene where the Orcs catapult several decapitated human heads onto the ground.
Arkham Asylum would not even necessitate an R rating. As stated, there are several horror films that only have PG-13 ratings these days. Arkham Asylum contains tons of gore, blood, guts, and sex. If it were rated as a film it would be for Violence and freighting images.
I guess I need to dig out my copy and read it again. I don't recall there being any sex in that story at all.
On topic of discussion. Yes, you can get away with quite a bit in a PG-13 film now.
