I think that's Peter's first direct encounter with Lizzy.

Peter no doubt underestimated the threat... I'm guessing it's the battle in the school leading into the sewers, which explains his dirty costume. I doubt his costume would be that intact if it was the final battle.

He could have fallen and cut his face, because those large cuts in his chest are definitely from The Lizard.

The belt wouldn't fit with this costume. It would just look awkward. People have made manips of this costume with a belt and it looks stupid.
